Uninsured Services

Below is a list of uninsured services supplied by our office. The costs are based on recommendations provided by the Ontario Medical Association. If you have any questions please speak directly to your physician.

Office fees not covered under OHIP

Release of chart: $25
Letters /Forms (signature only$10.00): $25
Sick notes/return to school/daycare: $10
Imm. Record replaced: $10
No show appointments: $50

No OHIP Coverage charges

Initial visit: $80
Regular check up: $40
Regular check up w/vaccinations: $50
Sick visit appointment: $40
Immunization only: $15
Consultation fee: $180.00

Please note if your child’s health card is soon to be expired please renew at Service Canada either on line or in person.
If your child’s health card has not been renewed within a reasonable time frame you will be charged for the appointment.
