Family physicians and other pediatricians are free to fax referrals to 416 497 0712.

Our doctors may take a referral as they come in. 

If you’re sending a referral, please be sure to include:

  • patient name 
  • health card number 
  • reason for referral  
  • relevant lab data 
  • relevant growth data (current and previous height & weight)
